This Joint Last Will and Testament ("Will") is made on this day of , 20 , by and between:

We hereby revoke all former Wills and codicils made by us, either jointly or severally.
We hereby appoint as Executor of this Will. If this person is unable or unwilling to act, we appoint as alternate Executor.
