Sports League Release of Liability

I, the undersigned participant, acknowledge and agree to the following terms regarding my voluntary participation in the activities, practices, games, and events (“Activities”) organized by the Sports League:

Assumption of Risk

I understand that participation in the Activities involves inherited risks, such as injury, illness, or other damages. I voluntarily accept and assume all risks associated with my participation.

Release and Waiver

I hereby release and discharge the Sports League, its officers, staff, representatives, coaches, volunteers, sponsors, and agents from any and all liability, claims, demands, or causes of action for injuries or damages resulting from my participation, whether caused by negligence or otherwise.

Medical Treatment Authorization

In the event of injury or illness, I authorize the Sports League to obtain necessary medical treatment. I understand that I am responsible for any costs incurred for such treatment.

Indemnification

I agree to indemnify and hold harmless the Sports League from any claims, demands, or damages arising from my participation in Activities.

Certification

• I have read this Release of Liability in its entirety.
• I fully understand its terms and voluntarily agree to them.
• I am of legal age or have obtained parental/guardian consent as indicated below.
