I, the undersigned, acknowledge and fully understand that participation in private swim lessons involves inherent risks, and I assume all responsibility for personal injury, accidents, or illnesses. I hereby release and discharge the swim instructor and all associates from any and all liability arising from participation in these activities.

I certify that the participant is physically fit to take part in private swim lessons.
