I, the undersigned, acknowledge that I have voluntarily chosen to rent and use the equipment listed above. I understand that use of this equipment carries certain inherent risks and dangers, and I use it at my own risk. I agree to release, indemnify, and hold harmless the rental provider, its employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while using or in possession of the equipment.
