Release of Liability Form for Volunteer Activities

This Release of Liability ("Release") is made by the undersigned participant regarding volunteer activities held by:

1. Acknowledgment of Risk

I understand and acknowledge that volunteering for the above activity may involve certain risks, including but not limited to physical injury, property damage, or other unforeseeable incidents.

2. Voluntary Participation

I confirm that my participation is voluntary and that I am physically fit and able to participate in the activities.

3. Release and Waiver

I hereby release, waive, and hold harmless the above-named organization, its officers, employees, and agents from any and all liability for injury, loss, or damage resulting from my participation.

4. Medical Treatment

I consent to receive medical attention in the event of injury or illness, and accept responsibility for all associated costs.

5. General Provisions

I certify that I have read and understand this Release. This Release is binding upon me and my heirs, family, estate, and representatives.
