I, the undersigned, acknowledge that participation in the dog walking group activity involves certain risks, including but not limited to injury to myself, my dog, or others, as well as property damage.
I certify that my dog is up to date on vaccinations and is not known to be aggressive toward people or other animals.
I agree to be responsible for the actions and wellbeing of my dog at all times during group activities.
I will follow all group guidelines and instructions provided by the group organizers.
I release the organizers, volunteers, and participants from any liability for injury or damage sustained as a result of participation.
