Music Band Tour Excursion
Liability Waiver

This Liability Waiver ("Agreement") is entered into by the undersigned Participant for participation in the Music Band Tour Excursion ("Activity").

I understand and acknowledge that participating in the Activity involves risks, which may include travel, transportation, physical activity, and other possible hazards. I voluntarily assume all risks associated with the Activity.

I release, waive, and discharge the organizers, band members, tour operators, agents, and affiliated parties from any and all liability for injury, loss, or damage incurred as a result of my participation in the Activity, whether caused by negligence or otherwise, to the fullest extent permitted by law.

In the event of injury or illness while participating, I authorize the organizers to seek necessary medical treatment on my behalf.

• I certify that I am physically fit for participation.
• I have read, understood, and agree to this Agreement.
• This Agreement is binding upon me, my heirs, executors, and assigns.
