I, the undersigned Model, hereby grant permission to the Instructor/Organization named above and their students to use my likeness, image, and physical representation in drawings, paintings, photographs, and/or other works created during the art class sessions referenced above.
I confirm that I am at least 18 years of age, or, if under 18, that I have obtained consent from a parent/legal guardian as indicated below.
I understand that the resulting artworks may be used for educational purposes, portfolio display, exhibitions, and related activities, without compensation to me. I understand that my name will not be publicly disclosed without additional consent.
