This Limited Use Art Model Release Agreement (“Agreement”) is made as of by and between the undersigned Model (“Model”) and Artist (“Artist”).
The Model grants the Artist permission to use photographs, images, or likeness of the Model, taken on at (“Images”) for the following limited purpose(s):
The Artist may use the Images only for the purpose(s) described above. Any additional use requires separate written consent from the Model.
The Model will receive the following compensation:
The Model retains all rights not specifically granted in this Agreement. The Artist agrees not to license, sell, or otherwise use the Images beyond the specified purposes.
The Model releases the Artist from any claims arising out of the use of the Images in accordance with this Agreement.
This Agreement is effective as of the date signed and remains in force for the specific uses listed above unless terminated by written agreement of both parties.
This document constitutes the entire agreement between the parties.
