Art Therapy Session Confidentiality Agreement

This Confidentiality Agreement (“Agreement”) is made between the undersigned client and the art therapist, respecting all information shared during art therapy sessions.

1. Confidentiality

All information and artwork shared in the art therapy sessions will be kept strictly confidential.

2. Exceptions to Confidentiality

Confidentiality may be broken only under the following circumstances:

• When there is reason to believe the client is at risk of harm to self or others
• When there is suspicion of abuse or neglect as required by law
• When ordered by a court of law

3. Consent to Release Information

No information will be released to a third party without written consent from the client except as stated above.

4. Storage of Records

Session notes and artwork will be securely stored and only accessible to the therapist.

5. Acknowledgment

By signing below, the client acknowledges understanding and consent to the terms of this confidentiality agreement.
