Art Therapy Confidentiality Agreement

1. I understand that any artwork, discussions, or personal information shared during therapy sessions will remain confidential between myself and the art therapist, except as otherwise required by law.
2. Confidentiality may be broken if:
   
   • There is a risk of harm to myself or others.
   • If there is knowledge or suspicion of abuse or neglect of a minor, elder, or dependent adult.
   • Required by court order or legal mandate.
3. My artwork and session information will not be used for educational or publication purposes without my written consent.
4. I understand that electronic communications (e.g., email, telehealth) may have limits to confidentiality.
