I hereby authorize and consent to the collection, use, and verification of my personal information for the purpose of conducting a background check to determine my eligibility for employment aboard the vessel. I understand that this may include inquiries into my criminal history, employment history, educational background, and other relevant information.

I release all persons and organizations from any liability or responsibility for providing, or receiving such information as required or permitted by law.
