I acknowledge that I am voluntarily participating as a volunteer at the homeless shelter. I understand and agree to the following:

• I will comply with all rules and regulations set forth by the organization and its staff.
• I understand there are certain risks associated with volunteering in this environment.
• I assume all risks of injury or harm in connection with my volunteer participation, and I release the shelter, its staff, and affiliates from all liability.
• I attest that I am physically able and prepared to provide volunteer services at the shelter.
• If I am under 18 years old, I have obtained permission from my legal guardian.
