I acknowledge that I am volunteering for disaster relief activities and understand the risks involved in such activities. I agree to follow all safety instructions, respect the directions of supervisors, and act responsibly to minimize risks to myself and others.

I confirm that all information provided is accurate. I understand that participation is voluntary and I may withdraw at any time.

Waiver and Release of Liability

I hereby release and hold harmless the organizing entity, its officers, employees, and partners from any and all liability for injuries, losses, or damages that may occur as a result of my participation in disaster relief activities, except as may be caused by gross negligence.

I understand that this waiver applies to all claims resulting from participation, whether known or unknown.
