This Model Release and Waiver Agreement ("Agreement") is entered into between:
("Model") and
("Photographer").
The Model hereby grants the Photographer and its legal representatives, licensees, and assigns, the absolute and irrevocable right and permission to use, adapt, modify, reproduce, publish, and distribute images or videos (the "Images") taken of the Model for any purpose whatsoever, including but not limited to advertising, promotion, marketing, and commercial uses, in any and all media now known or hereafter developed, without restriction as to alteration.
The Model waives any right to inspect or approve the final Images or the use to which they may be applied. The Model releases and discharges the Photographer, its representatives, assignees, employees, and any person acting under its authority from any and all claims, demands, or liabilities arising out of or in connection with the use of the Images, including but not limited to any claims for libel or invasion of privacy.
The Model acknowledges that they have received good and valuable consideration for this release. No further compensation is or will be due.

The Model affirms that they are of legal age and have the full legal capacity to execute this Agreement. If the Model is under the age of 18, a parent or legal guardian must also sign below.
