I hereby grant permission to __________________________ ("Photographer") to photograph me and to use my likeness in all forms and media for any lawful purposes, including publicity and advertising. I waive any right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears.
I release and discharge the Photographer from all claims and liability relating to said photographs.
