This form grants permission for the use and distribution of photographs, video recordings, and other media in which you may appear as a board member of our nonprofit organization.
I understand and agree that photographs, video, audio or other media of me taken by the organization may be used in print, online, and/or other public media for promotional and educational purposes. I acknowledge that the organization's right to use these images and recordings is perpetual.
