I understand that as a participant in this program, I may learn personal information about other individuals and families. I agree to keep all such information confidential and not share it outside of the program except as required by law.

I agree not to discuss, disclose, or post confidential information about members, volunteers, staff, or the program in any form, including in conversation, in writing, or online.

I understand that violating this agreement may result in disciplinary action, up to and including removal from the program.
