I hereby grant permission to the nonprofit organization and its representatives, to take photographs, videos, or other media of my child or myself during program activities. I authorize the use, publication, and distribution of these media for any lawful purpose including, but not limited to, educational, promotional, and fundraising materials, without compensation.
I understand that participation in the program may involve physical or other activities that could result in injury. I release and hold harmless the nonprofit organization, staff, volunteers, and affiliates from any and all liability, damages, or claims arising from participation in the program.
