COVID-19 Liability Waiver

For Nonprofit Youth Events

In consideration for being allowed to participate in events and activities hosted by (the "Nonprofit"), the undersigned acknowledges, appreciates, and agrees that:

• I am aware of the highly contagious nature of COVID-19 and the risk that I and/or my child may be exposed to or contract COVID-19 by attending or participating in the Nonprofit's events or activities.
• I understand that such exposure or infection may result in illness, personal injury, permanent disability, or death to myself or my child.
• I voluntarily assume all risks related to exposure to COVID-19 and accept sole responsibility for any injury, loss, or damage that may occur as a result of such participation.
• I acknowledge that the Nonprofit has put in place preventative measures to reduce the spread of COVID-19, but cannot guarantee that I or my child will not be exposed to the virus.
• I agree to comply with all precautionary measures and instructions imposed by the Nonprofit, including but not limited to mask-wearing, social distancing, hand hygiene, and temperature checks, if required.
• I release, waive, discharge, and hold harmless the Nonprofit, its directors, officers, employees, volunteers, agents, and representatives from any and all claims, liabilities, actions, damages, costs, or expenses of any kind arising out of or relating to COVID-19 exposure or infection resulting from participation in Nonprofit youth events.

I certify that I am the participant or their legal guardian, and that I have read this agreement, fully understand its terms, and sign it freely and voluntarily.
