I, the undersigned volunteer, acknowledge and agree that participation in disaster relief efforts with (the “Organization”) may involve activities that could result in injury, death, personal property damage, or loss. I hereby voluntarily assume all risks associated with my participation.
I release, waive, discharge, and hold harmless the Organization, its officers, directors, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action associated with or arising from my participation as a volunteer.
I have read this waiver and fully understand its terms. I sign it freely and voluntarily without any inducement.
Parent/Guardian Consent: I am the parent/legal guardian of the minor named above and consent to their participation. I agree to the terms of this waiver.
