Event Participation Liability Waiver Form

I acknowledge my voluntary participation in this nonprofit event and understand that such participation may involve certain risks.

I, hereby, release and hold harmless the organizers, their officers, employees, and volunteers from any and all liability, claims, or demands for personal injury, illness, or property damage which may arise from or in connection with my participation.

I certify that I am physically fit and able to participate in this event.
