This Release and Waiver of Liability (the "Release") is executed by the undersigned volunteer ("Volunteer") in favor of [Nonprofit Name], its directors, officers, employees, agents, and successors ("Nonprofit").
The Volunteer desires to engage in nonprofit activities and hereby voluntarily executes this Release under the following terms:
The Volunteer releases, waives, and forever discharges the Nonprofit from any and all liability, claims, and demands in connection with any loss or damage related to participation in activities with the Nonprofit.
The Volunteer waives all claims against the Nonprofit for any first aid, medical treatment, or services rendered in connection with volunteer activities.
The Volunteer understands the activities may involve certain risks and voluntarily assumes full responsibility for any injury, accident, or loss.
The Volunteer acknowledges that the Nonprofit does not carry or provide health, medical, or disability insurance for volunteers.
The Volunteer grants the Nonprofit permission to use photographs, video, or audio recordings in any medium for purposes of publicity or promotion.
By signing below, the Volunteer has read, understands, and voluntarily signs this Release.
