I hereby grant permission to use my likeness, image, and illustrations for the above-mentioned advertising campaign, in any and all media, for any purpose consistent with the campaign, without payment or any other consideration.

I understand that these materials will become the property of the organization and will not be returned. I hereby waive any right to inspect or approve the finished product.
