Agreement Terms

  1. The Employee confirms the return of the above company-owned mobile device and all associated accessories as listed above.
  2. The device is being returned in the condition it was initially provided, reasonable wear and tear excepted.
  3. Any damage, loss, or missing accessories may result in liability as per the company's device policy.
  4. The Employee acknowledges that all company data has been deleted from the device and it is no longer accessible to unauthorized persons.
  5. This agreement is effective as of the date signed below.
