This Media Release Agreement ("Agreement") is entered into by and between:

The undersigned grants permission to the above-named sports team/organization to use photographs, videos, and/or audio recordings (collectively, "Media") of the Participant for the purposes of publicity, promotion, and media coverage.
The Participant hereby authorizes the team/organization to use, reproduce, publish, and distribute Media in any and all formats, including but not limited to print, digital, social media, and promotional materials.
The Participant waives any right to inspect or approve the finished product, regardless of format, and waives all rights to royalties or other compensation related to the use of the Media.
The Participant releases the team/organization, its representatives, and assigns from all claims, demands, and liabilities whatsoever in connection with the use of the Media.
This Agreement is effective as of the date signed below and remains in effect unless revoked in writing by the Participant or legal guardian.
