Media Release

I hereby grant permission to the organizers of the dance recital to record, photograph, and use the name, image, voice, and likeness of the participant in any media forms for promotional, educational, or documentation purposes.

Liability Waiver

I acknowledge and accept all risks associated with participation in the dance recital. I release the organizers, staff, and associated parties from any and all liability, claims, demands, or causes of action that may arise from participation in this event.
