Media Release

I hereby grant permission to the organizers and affiliates of the fitness class to take and use photographs, video, or digital images of me during class activities for promotional, educational, or any other lawful purpose.

Liability Waiver

I acknowledge that participation in fitness classes involves physical activity and that I do so at my own risk. I hereby release the organizers, instructors, and facility from any and all liability, claims, demands, or causes of action that may arise from my participation in these activities.
