I acknowledge and agree that by accessing the set, locations, or facilities of the film production named above, I assume all risks associated with my presence and activities. I hereby waive, release, and discharge the production company, its employees, agents, and affiliates from any and all liability, claims, and causes of action arising from or related to personal injury, property damage, or loss that may occur during my presence at or participation in the production, except as may be caused by gross negligence or willful misconduct.
I understand that media and photography may occur on set and consent to being photographed, filmed, or recorded for production purposes.
