I acknowledge that I have voluntarily chosen to participate in personal training sessions, classes, or programs provided by the personal trainer. I am aware that participation may involve risks, including but not limited to physical injury, damage to property, or even death.
I understand and agree that by signing this Waiver, I assume full responsibility for any and all risks of personal injury or other loss that may be sustained by me as a result of participation, whether caused by negligence or otherwise.
I knowingly and voluntarily release, waive, and discharge the personal trainer and any associated facility from any claim, demand, or cause of action arising out of or related to my participation.
I certify that I am physically fit and sufficiently trained to participate in personal training activities and have not been advised otherwise by a qualified health professional.
I acknowledge that I have read, understand, and fully accept the terms of this waiver.
