I, the undersigned, acknowledge and agree to participate in the wedding party transportation services. I hereby release and hold harmless the transportation provider, its drivers, owners, agents, and affiliates from any and all liability for injuries, damages, losses, or claims arising from or in connection with my participation.
I understand that seat belts should be worn at all times, no standing or moving while the vehicle is in motion, and all instructions from the driver must be followed. I voluntarily assume all risks related to transportation.
