Volunteer Driver Passenger Waiver and Release Form
I, the undersigned passenger, hereby agree to participate as a passenger in a vehicle driven by a volunteer driver associated with the organization and acknowledge and agree to the following terms and conditions:
- I understand that participation as a passenger is voluntary and may involve certain risks, including but not limited to injury or property damage.
- I hereby waive, release, and discharge the organization, its directors, officers, employees, agents, and volunteer drivers from any and all liability, claims, demands, or causes of action that may arise from my participation as a passenger.
- This waiver and release apply to any injury, loss, or damage, whether caused by negligence or otherwise.
- I affirm that I am at least 18 years of age or have permission from a parent or guardian to participate.
