I, the undersigned participant, acknowledge that participation in the Adult Softball League carries with it the potential for certain risks, including but not limited to physical injury, property damage, or even death. I certify that I am physically fit and sufficiently prepared for participation in this program.
In consideration of being permitted to participate in any way in the Adult Softball League, I hereby assume all risks and waive, release, and discharge any and all claims for damages for personal injury, death, or property damage which may have or which may hereafter accrue as a result of participation in said event.
This waiver is intended to discharge in advance the organizers, sponsors, officials, and all affiliated persons or entities from any and all liability arising out of or connected in any way with my participation.
I have carefully read, understood, and voluntarily sign this waiver and release of liability.
