Consent & Waiver

I, the undersigned parent/legal guardian, hereby grant permission for my child to participate in the Esports Tournament.

• I acknowledge and accept all risk of injury or harm that may occur as a result of participation.
• I release the tournament organizers, sponsors, coaches, and venue from any and all liability arising out of my child’s participation.
• I authorize medical treatment to my child in case of emergency if I cannot be contacted.
• I give permission for the tournament organizers to use photographs, video, or likeness of my child for promotional purposes.
