I acknowledge that participation in Pickleball club activities involves physical risk. I voluntarily assume full responsibility for any risk, injury, or damage associated with my participation.
In consideration for being permitted to participate in Pickleball Club events, I voluntarily agree to release, discharge, and hold harmless the club, its officers, instructors, and volunteers from any and all claims, damages, or liabilities arising from my participation.
I am physically fit to participate in this event. I have read and fully understand this Release & Indemnity Agreement.
