Youth Soccer Tournament Liability Waiver

In consideration of my child’s participation in the Youth Soccer Tournament (“Event”), I, the undersigned, as parent or legal guardian of the participant, acknowledge and agree to the following:

  1. I am aware that participating in a soccer tournament involves inherent risks, including injury, illness, or property damage, which may result from my child's participation.
  2. I voluntarily assume all risks associated with my child’s attendance and participation in the Event.
  3. I hereby release and hold harmless the organizers, sponsors, coaches, officials, volunteers, and facility owners (“Released Parties”) from any and all liability, claims, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury that may be sustained by my child while participating in the Event.
  4. I certify that my child is physically able to participate in the Event and that I will be responsible for any medical expenses incurred as a result of injuries from participation.
  5. In the event of an emergency, I authorize the Event organizers to secure medical treatment for my child.
