Adult Hockey League Medical Release and Waiver

I, the undersigned participant, acknowledge that participation in the Adult Hockey League involves a risk of injury or harm. I voluntarily accept and assume all risks associated with my participation. I certify that I am physically fit to participate and have not been advised otherwise by a qualified medical professional.

Medical Information Authorization

I authorize the league organizers, employees, volunteers, or agents to obtain or provide any emergency medical treatment that may be necessary in the event of illness or injury while participating in league activities.

Waiver and Release of Liability

• I hereby release and discharge the league, its directors, officers, employees, agents, and volunteers from any and all liability, claims, or causes of action for injuries, illness, or damages arising out of or related to my participation.
• I understand and agree that this waiver and release is binding on me and my heirs, executors, administrators, and assigns.
