I acknowledge that by participating in team/group sports and related activities, there may be an increased risk of exposure to the COVID-19 virus. I agree to abide by all guidelines and protocols set forth by the organization/club/venue to reduce the risk of transmission.

I have read and understood the above statement.
I confirm that I (or my child) do not currently have symptoms of COVID-19 or a recent positive test.
I agree to notify the organization if I (or my child) develop symptoms or test positive for COVID-19.
