I, the undersigned, as the parent or legal guardian of the above-named participant, acknowledge and fully understand that participation in the Youth Cheerleading Event involves inherent risks of physical injury. I willingly assume full responsibility for any and all risks and hazards, whether known or unknown, related to the event.
I hereby release and hold harmless the event organizers, sponsors, staff, coaches, volunteers, and all affiliated parties from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by the participant while participating in, or traveling to or from this event.
I certify that the participant is in good health and has no medical condition that would prevent safe participation in the event. I authorize emergency medical treatment if necessary.
