Waiver and Release of Liability
I, the undersigned parent or legal guardian of the above-named participant, acknowledge that participation in swimming lessons involves inherent risks, including but not limited to the risk of injury, illness, and drowning. I voluntarily assume all such risks and agree to release, indemnify, and hold harmless the organizers, instructors, facility, and their representatives from any and all claims or liability arising out of or related to participation in swimming lessons. I certify that my child is in good health and has my permission to participate. I have read and understood this waiver and release.
