Assumption of Risk & Release of Liability

I, the undersigned, acknowledge and fully understand that participation in the golf tournament involves risks of serious injury, including bodily harm, permanent disability, and even death, as well as property damage and other losses.

I voluntarily assume all such risks and take full responsibility for my participation. I hereby release and hold harmless the organizers, sponsors, volunteers, and any associated facilities from any and all claims, liabilities, injuries, damages, or losses incurred as a result of my participation.

I have read, understood, and agree to the terms of this liability waiver.
