Assumption of Risk & Release of Liability
I, the undersigned, acknowledge that participation in the CrossFit Challenge Event involves inherent risks, including but not limited to falls, physical injury, or in rare cases, death. By signing below, I voluntarily assume all such risks and agree to release, waive, discharge, and hold harmless the event organizers, sponsors, coaches, volunteers, and the facility from any and all liability for injuries, losses, or damages resulting from my participation.
Medical Authorization
I certify that I am physically fit and have not been advised otherwise by a qualified medical professional. In the event of an emergency, I authorize event officials to secure medical treatment on my behalf.
Photo/Media Release
I grant permission to use my image, likeness, and name in any photographs, recordings, or media coverage associated with this event, without compensation.
