By signing below, I acknowledge that I am voluntarily participating in the martial arts tournament. I fully understand and accept all risks associated with my participation, including but not limited to physical injury, property damage, or other loss that may occur as a result of my participation.
I, for myself, my heirs, personal representatives, or assigns, hereby release, waive, discharge and covenant not to sue the organizers, sponsors, instructors, referees, volunteers, facility, or any associated personnel of this tournament for any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me during or in connection with my participation in this event.
I understand and agree to abide by all tournament rules and regulations. I certify that I am physically fit and sufficiently trained for participation. In the event of an emergency, I authorize medical personnel to treat me if necessary.
I have read, understand, and agree to the terms stated in this release and waiver.
