High School Basketball Player Conduct Acknowledgement

I understand that as a member of the high school basketball team, my conduct reflects on myself, my team, and my school. I acknowledge the following guidelines and agree to abide by them throughout the basketball season:

1. I will demonstrate sportsmanship and respect toward teammates, coaches, opponents, officials, and spectators at all times.
2. I will attend all practices, games, and team activities, unless excused by my coach.
3. I will maintain academic eligibility in accordance with school policy.
4. I will follow all school rules and team policies, including dress code and attendance requirements.
5. I will refrain from the use of tobacco, alcohol, drugs, or any illegal substances.
6. I will communicate honestly and promptly with coaches regarding any issues or concerns.
7. I understand that failure to comply with these expectations may result in disciplinary action, including suspension or removal from the team.
