Recreational League Player Conduct Waiver

By signing below, I acknowledge that I have read, understand, and agree to abide by the Recreational League’s Player Conduct Policies as outlined below. I understand that failure to comply may result in disciplinary action including suspension or expulsion from the league.

Player Conduct Code

• Respect all players, coaches, officials, and spectators at all times.
• Refrain from the use of foul or abusive language and gestures.
• Play within the rules and uphold the principles of fair play and sportsmanship.
• Do not engage in any form of physical or verbal abuse.
• Accept decisions made by officials without protest.
• Avoid any behavior that could endanger others.
• Maintain a positive attitude on and off the field/court.

Assumption of Risk & Release of Liability

I understand that participation in recreational sports involves inherent risks of injury. I voluntarily assume all risks associated with my involvement and release the league, its organizers, and affiliates from any liability to the fullest extent permitted by law.

Agreement

I certify that I am at least 18 years old, or have my parent/guardian’s consent if under 18. I have read this waiver and agree to its terms.
