Event Transportation Parental Consent Form

I, the parent or legal guardian of the above-named student, give consent for my child to be transported to and from the above-named event by the organization-approved transportation. I understand that all reasonable safety precautions will be taken. In case of an emergency, I authorize medical treatment as necessary.
