I, the undersigned parent or guardian, give permission for my child identified above to participate in the specified school field trip and authorize the use of school-approved transportation. I understand the nature of the trip and the means of transportation that will be used.
I acknowledge and accept the potential risks involved in transportation to and from the field trip destination. I hereby release and hold harmless the school, its employees, agents, and volunteers from any liability arising from transportation in connection with this event.
