Church Mission Trip Liability Waiver

This Liability Waiver must be completed and signed by every participant (or their parent/guardian if under 18) before attending or participating in the mission trip organized by the church. By signing this form, you acknowledge and agree to the terms described below.

Liability Release

I hereby acknowledge that participation in the church mission trip involves certain risks, including but not limited to travel, physical activities, work, and interactions with new environments and people. I voluntarily assume all such risks. I release, indemnify, and hold harmless the church, its staff, leaders, volunteers, agents, and affiliates from any and all claims, losses, or liabilities for injuries, damages, or expenses that may arise during participation in this event or as a result of it.

Medical Authorization

In the event of any medical emergency, I authorize church representatives to obtain medical treatment for myself (or my child) at my expense, and hereby release them from any liability in connection with such decisions.
