I acknowledge that skateboarding is a potentially hazardous activity. I understand and accept all risks associated with participating in the Teen Skateboarding Event, including but not limited to falls, collisions, or injuries.

Waiver & Release:

I hereby release the event organizers, sponsors, volunteers, and property owners from any liability for injuries, damages, or losses incurred as a result of my participation in the event.

Medical Treatment:

In the event of an injury or medical emergency, I authorize the event organizers to seek medical attention on my behalf.

Parental Consent (Required if under 18):

I am the parent or legal guardian of the above-named participant and consent to their participation. I accept all terms of this liability waiver.
