Amateur Baseball League Player Waiver

I, the undersigned, wish to participate in the Amateur Baseball League (the "League"). I acknowledge that baseball is a physical sport that may involve inherent risks of injury, including but not limited to: slips, falls, being struck by equipment or balls, and collisions with other players.

Assumption of Risk

I voluntarily assume all risks associated with my participation in League activities, including practices, games, and related events.

Release of Liability

I hereby release, waive, and discharge the League, its organizers, coaches, volunteers, sponsors, and associated facilities from any and all claims, demands, or causes of action resulting from injury or damages sustained during League activities, whether caused by negligence or otherwise, to the fullest extent allowed by law.

Medical Authorization

In the event of injury or illness, I authorize the League and its representatives to seek and obtain reasonable medical treatment on my behalf if I am unable to do so myself.

Agreement and Certification

I certify that I have read this waiver and fully understand its contents. I am at least 18 years old (or have obtained consent from my legal guardian if under 18).
